More Healthcare Transformation Grants Available to NY Providers
After issuing a Request for Applications (RFA) for the Statewide Health Care Facility Transformation Program IV and V funding in support of Health Information Technology, Cybersecurity, and Telehealth Transformation last week, the New York State Department of Health (NYSDOH) and Dormitory Authority of the State of New York (DASNY) have announced another opportunity for the state’s healthcare providers to receive vital government funding under the program.
On January 9, the NYSDOH issued an RFA to apply for funding in support of projects directly related to Residential and Community-based Alternatives to the Traditional Model of Nursing Home Care. Eligible expenses include construction and renovation costs, asset acquisitions, equipment costs and consultant fees.
To qualify for a piece of the $50 million of grants, applicants must be able to prove that the project(s) will:
- transform, redesign and strengthen the quality and innovation of residential healthcare services and programs, such as the conversion of a nursing home into separate residential-style homes
- improve health outcomes and patient experience
- create and contribute to a financially sustainable system of care
- preserve or expand the ongoing availability of essential healthcare services to communities served by the applicant
Applications are due by April 9, 2024 at 4:00 p.m. ET. and must include the above information, as well as an estimate of the total costs of carrying out the transformation activities and the ability to secure potential funding for such costs.
Multiple applications can be submitted by eligible organizations, which include:
- Hospitals
- Residential Health Care Facilities
- Adult Care Facilities
- Diagnostic and Treatment Centers
- Clinics
- Children’s Residential Treatment Facilities
- Assisted Living Programs
- Home Care Providers
- Primary Care Providers
- Hospices
- Community-Based Programs funded under the Office of Mental Health, the Office of Addiction Services and Supports, the Office for People with Developmental Disabilities, or through local governments
- Independent Practice Associations or Organizations
- Mental Health Clinics
- Alcohol and Substance use disorder treatment clinics
- Day Programs
All applicants must be prequalified in the New York State Grants Gateway or Statewide Financial System (SFS) before submitting a proposal.
